Send one request for an in-person or online consultation with SKIN LAB CLINIC. Residents and visitors to Japan can apply.
How booking works
You only need to submit the form once. The clinic—not VisaSHOGUN—confirms your appointment and makes all medical decisions.

The clinic reviews it
SKIN LAB CLINIC checks your request during clinic hours. Submission does not guarantee a prescription.
Receive confirmation
The clinic contacts you by email or WhatsApp. If your preferred times are unavailable, it will suggest alternatives.

For online consultations, live English telephone support is not guaranteed and a translation app may be required. In-person consultation is recommended if you need language support.
International patient prices
One-month supply containing four pens. The doctor decides whether treatment is appropriate and which dose may be prescribed.

What happens after you submit?
- SKIN LAB CLINIC reviews your request during clinic hours.
- The clinic emails or messages you on WhatsApp with a confirmed appointment time.
- If none of your preferred times are available, the clinic sends alternative dates for you to choose from.
- Please do not visit the clinic or assume an online appointment is booked until you receive confirmation.
